Our social workers' job is to provide enhancement of the quality of life, and to the development of the full potential of each individual, group and community in our society. Our social workers are licensed professionals who are able to provide patients and their family peace of mind when it comes to choosing the right services in home health care.
They can also help you with discharge planning before your loved one has to go home from the hospital.
